New Patients

We are happy to see new patients that are self-referred although most patients are generally accepted on a referral basis from their attending physician. We know how stressful the initial diagnosis can be, and we strive to keep waiting time for an appointment to a minimum.

Initial Appointment

When you schedule your appointment, we request your medical records. We do this so we have correct information regarding your diagnosis, previous evaluation, management, and future care needs.

Time permitting, we mail you paperwork to complete before your first office visit. Or, you can download the information here. Please complete these forms and present the information to the registrar at check-in. If you do not receive this information in the mail, you will need to complete it when you arrive for your first visit, so allow enough time to do this (usually 30 minutes).

Insurance and Identification Cards
Please bring your insurance cards and picture identification so we can submit charges for payment to your insurance provider. Our staff will work with you before your first appointment to ensure you have the coverage you need. For more information, visit the Insurance and Billing section.

HMOs
Patients with HMO insurance plans must bring the referral from their primary care physician to their first visit.

Medication
Please bring a list of medications that you are currently taking (including dosages and directions for use) to be recorded in your chart. This includes over-the-counter remedies, vitamins, and herbal supplements.

Your First Visit
Please arrive 30 minutes before to your scheduled time so the registrar can obtain any additional information needed for your appointment. We are happy to help you complete any necessary forms.

Initial office visits usually take longer than follow-up visits. During this time, the physician will ask you questions about your present and past medical history, including any previous surgery and your family history, and perform a physical examination. The remainder of the visit is spent with the physician and clinical staff discussing your diagnosis, evaluation, and treatment options. Please feel free to ask questions at any time during this first visit.

Following this discussion, the physician and staff coordinate an individual care plan. Follow-up appointments are scheduled as needed according to your care plan.
